About Cook Children's Home Health
Meeting the diverse needs of kids in the comfort and convenience of their own home is our main priority. Our staff develops an individualized care plan for each patient and works with the family on a monthly, weekly or daily basis.
Specialized for Your Family
Each patient is assigned a Cook Children's Home Health care coordinator. This coordinator is a registered nurse who ensures that every family is treated with respect, that all care is coordinated and that communication between the family and providers is timely and accurate. Patients and their families may call on a pediatric home care nurse at any time. They are always available 24 hours a day, 7 days a week.
Cook Children's Home Health offers clinical services such as initial discharge planning, patient assessment, home care instruction and ongoing treatments and/or evaluation.
Home Health provides services through Cook Children's employees. We don't contract with outside providers. Our caregivers are Cook Children's employees. This helps us give patients and their families greater continuity and convenience as well as significantly lowering costs.
Home Health pharmacists, orthotic and prosthetic practitioners, nurses, social workers, respiratory therapists and occupational, physical and speech therapists are trained in the science of pediatric home care. All submit to an intensive interviewing process and receive thorough background checks. Cook Children's Home Health nurses are required to be PALS (Pediatric Advanced Life Support) certified and most have a minimum of five years' experience, and most rehab therapists have at least 15 years' experience.

Referrals may be made 24 hours a day, seven days a week. Office hours are from 8:30 a.m. to 5:30 p.m., Monday through Friday. After hours, an answering service will connect referring physicians or clinicians to the on-call nurse
